This is just to reconfirm My Practice Policies for patient’s appointments, procedures and payments again in 2025 to perpetuity.
All appointments require a full 24 hour cancellation to avoid a full office visit charge of $300.00.
If your appointment is at 12pm we require cancellation by 12 pm the full day prior to avoid the fees.
A patient that repeatedly cancels will be required to book their appointments with a deposit of $150.00.
No shows for any appointment will incur the full office visit fee. No exceptions.
All appointments will require a credit card on file at the time of scheduling an appointment.
No surgery will be scheduled without the 75% payment for the procedure. No surgery dates can be changed once scheduled.
No refunds for patient electing to cancel or reschedule a surgery once booked. Only a medical emergency, personal hospitalization will be allowed to do so.
Patient consultations are free with a scheduled paid procedure.
Otherwise the consultation fee is $350.00 and can be applied to a future surgery. Pre op appointments are included in the surgical fee. If any refund exceptions are made they will be with a deduction of $350.00 per each office visit that was included as part of a surgery as well as any materials used in any form of treatments.
If a patient is unsure, each consecutive appointment or additional consultation 
does incur a full consultation fee as we spend on average 45min -1 hour and we are very thorough and non-rushed in our time of consultation with each patient.
While we are sorry to have to institute such practices, it is very disrespectful of our time and availability that we do put aside for each patient and recently the office has been inundated with abuse of our prior tolerance to accommodate changes to appointments. Thank you for understanding as all other offices do the same and we now value these policies!
